As per the previous Halloween art, Sam devised this all by her lonesome. It was originally a desktop background made available exclusively for Patreon supporters.
This page also marks the second time that Ben has dressed up as the Heavy from Team Fortress 2, the first time being back in 2008, only this time the gun is more proportionate and he didn't bother to shave his hair off.
If it wasn't for the premature appearance of Dr. Ralph Smiffington the Third in the background, I'd have left the "non-canon" tag off as this occurs right in the middle of the interlude so it's entirely possible that Ben and Betty might have dressed up at this point! If you want to you can maybe pretend that this happened later on, sometime after episode 88...
Ben and Betty are dressed up for Halloween, Ben as the Heavy from Team Fortress 2 and Betty (rather appropriately) as the Medic, also from Team Fortress 2. Both are carrying guns appropriate for their attire. They are posing inside the Detulux compound, much to the bemusement of several onlookers which includes several henchmen, Mr. Pain, and the Captain. Also watching on is Snowball the cat, Mr. Pain's pet, who is peeking out from behind a set of pumpkins in the grass nearby that have been carved with scary faces for Halloween.
Hidden in the distance behind, spying from autumnal bushes, is Dr. Ralph Smiffington III who still hasn't been properly introduced into the comic yet. The not-so-good doctor hasn't realised that Ben's pet monster Bono is behind him and is sizing him up for lunch.
